Proteinuria

Mechanisms of Proteinuria

Sources: Chapters 26 and 32 of Skorecki, K et al. 2016; Rovin, B. 2021.

Types of Proteinuria

Albumin : Total Protein

Ratio of >0.4 suggests glomerular proteinuria rather than overflow or tubular proteinuria [Smith et al. 2012].

Post-Renal Proteinuria

Inflammation in the context of a UTI or nephrolithiasis. Typically non-albumin proteinuria. Unclear mechanism. [Rovin, B. 2021]

Additional Causes

Orthostatic (postural proteinuria when standing)

Fever

Exercise

What type(s) of proteinuria is detected on urine dipstick?

Albumin Only

What type of proteinuria did our patient have?

Urine Albumin: 400.0 mg/dL

  uACR = 4,395 mg/g

Urine Protein: >600.0 mg/dL

  uPCR = >6,593 mg/g

Glomerular +/- Tubular
